Intracavernosal Injections for Erectile Dysfunction: How Telehealth Training Works
If tablets haven’t been effective for erectile dysfunction (ED), intracavernosal injection (ICI) therapy can be a safe, reliable, and highly effective treatment option.
At Restorative Health Clinic, we teach intracavernosal injections via structured telehealth appointments, allowing patients across Australia to receive expert guidance, personalised dosing, and strong safety support — all from the privacy of their own home.
This article explains how the process works, why there are two appointments, what to expect from injection training, and how we prioritise safety at every step.
Why Intracavernosal Injection Therapy Requires Two Appointments
Before starting ICI therapy, it’s essential to ensure that this treatment is appropriate, safe, and suitable for you.
1. Initial Erectile Dysfunction Assessment
Your first appointment is a comprehensive erectile dysfunction assessment. During this consultation, your clinician will:
Take a detailed medical and sexual health history
Assess your current erectile function
Review medications and overall health
Consider contributing factors such as cardiovascular health, hormones, nerve function, or surgery
Discuss all available ED treatment options, not just injections
Only if intracavernosal injections are agreed upon together — by you and your clinician — do we proceed to the next step.
This ensures injection therapy is used appropriately and safely.
What Happens Next if You Proceed with ICI Therapy
If intracavernosal injections are the right option for you, we book your second appointment, which is a dedicated telehealth injection training session.
Before this appointment, we send you a carefully prepared injection starter pack, so you’re fully equipped and ready.
What’s Included in Your Injection Starter Pack
When you begin ICI therapy with Restorative Health Clinic, your starter pack includes everything you need to begin safely:
Three 2 mL vials
Two medication vials containing different formulations to trial
One vial of emergency medication (phenylephrine), used only if an erection lasts longer than expected
An Inject-Ease® auto-injector, designed to make injections smoother and more comfortable
Single-use syringes and needles specifically for penile injection therapy
A prescription for pseudoephedrine 60 mg tablets, which can help reduce a prolonged erection
Our goal is to ensure you feel prepared, supported, and confident before your first dose.
Your Telehealth Injection Training Appointment
Your second appointment is your telehealth injection training session.
During this appointment, one of our experienced clinicians will guide you through your first injection in real time. You are not expected to figure this out on your own.
We cover:
How to draw up the medication safely
How to use the Inject-Ease device
Correct injection sites
Hygiene and safe technique
How to monitor and record your response
What to do if the erection lasts longer than expected
We stay with you throughout the process so you feel confident and supported.
What Happens After Your First Injection
After your first test dose, you’ll monitor the erection and contact your clinician at the two-hour mark.
At that point:
You update us on firmness and duration
We advise you on the next dose to trial, if needed
Everyone responds differently to injection therapy. For safety reasons, we always start with a low dose and adjust gradually.
For most patients, it takes two to four test doses to find the ideal medication formulation and dose.
What If an Erection Lasts Too Long?
Prolonged erections (priapism) are uncommon when dosing is managed correctly, but safety planning is essential.
You’ll receive clear written and verbal instructions, including:
If the erection lasts longer than two hours, take two 60 mg pseudoephedrine tablets
Contact your clinician at the two-hour mark
If the erection hasn’t started to settle by three hours, take another two pseudoephedrine tablets
If the erection reaches four hours and hasn’t settled, we may guide you to use the emergency phenylephrine vial or, in rare cases, attend the nearest Emergency Department
You are never left to manage this alone — our team is available to guide you.
